L’emergenza di queste ore non è solo frutto della contingenza, ma un problema strutturale vecchio di almeno quattro decenni. Tutti i numeri del sistema sanitario italiano, considerato ancora tra i migliori al mondo
Negli ultimi giorni il dibattito pubblico italiano ha imparato a fare i conti con la locuzione medicina delle catastrofi, l’ambito scientifico che si occupa di mettere a punto una risposta sanitaria adeguata di fronte a situazioni emergenziali e alla conseguente scarsità di risorse mediche. Diverse testimonianze giornalistiche raccontano di un sistema sanitario pesantemente sotto stress, con reparti di terapia intensiva sull’orlo del collasso e dolorose scelte sui pazienti da intubare.
All’allarme lanciato dai media si è aggiunta in queste ore l’apprensione del sindaco di Bergamo Giorgio Gori, che su Twitter ha parlato di “pazienti lasciati morire” perché non possono essere trattati. Al momento non esistono dati certi sulla saturazione delle strutture lombarde e numerose voci mediche escludono apertamente simili ricostruzioni, ma in nessun caso i pazienti sarebbero comunque “lasciati morire”, dal momento che anche in assenza di respiratori sono previste tutte le cure necessarie.
Ciò che sappiamo con certezza, invece, è che lo stato in cui versa oggi la sanità italiana non è unicamente frutto della contingenza. È un problema strutturale, piuttosto, figlio di precise scelte di finanza pubblica, che nell’arco di 40 anni hanno contribuito a indebolire un servizio sanitario considerato, nonostante tutto, ancora tra i migliori al mondo.
I numeri della sanità italiana
Nel 2018 l’Italia ha speso per il sistema sanitario nazionale l’8,8% del Pil, una percentuale che scende al 6,5% considerando solo gli investimenti pubblici. Facciamo peggio di Stati Uniti (14,3%), Germania (9,5%), Francia (9,3%) e Regno Unito (7,5%), ma sostanzialmente in linea con la media Ocse, ferma al 6,6%. Sotto di noi solo i paesi dell’Europa orientale, Spagna, Portogallo e Grecia.
In numeri assoluti ciò si traduce in un esborso per lo stato di 2.326 euro a persona (2mila meno della Germania), complessivamente 8,8 miliardi più rispetto al 2010. Un tasso di crescita dello 0,90%, dunque, che con l’inflazione media annua all’1,07% si traduce in un definanziamento di 37 miliardi. La Fondazione Gimbe calcola che il grosso dei tagli sia avvenuto tra il 2010 e il 2015 (governi Berlusconi e Monti), con circa 25 miliardi di euro trattenuti dalle finanziarie del periodo, mentre i restanti 12 miliardi sono serviti per l’attuazione degli obiettivi di finanza pubblica tra il 2015 e il 2019 (governi Letta, Renzi, Gentiloni, Conte).
Come si legge nell’annuale relazione della Corte dei Conti, la frenata più importante è arrivata dagli investimenti degli enti locali (-48% tra il 2009 e il 2017)e dalla spesa per le risorse umane (-5,3%), una combinazione che in termini pratici si ripercuote sulla quantità e sull’ammodernamento delle apparecchiature, oltre che sulla disponibilità di personale dipendente, calato nel periodo preso in considerazione di 46mila unità (tra cui 8mila medici e 13mila infermieri).** **I mancati investimenti si fanno sentire soprattutto nel sud Italia, dove tutte le regioni (eccezion fatta per il Molise) spendono meno della media nazionale.
I numeri negli ospedali
I dati più affidabili per aiutarci a capire ciò che sta realmente accadendo negli ospedali italiani arrivano dall’annuario statistico del Servizio sanitario nazionale e sono aggiornati all’anno 2017.I posti letto complessivamente disponibili nelle strutture pubbliche sono 151.646 (2,5 ogni mille abitanti), che sommate alle oltre 40mila unità incluse in strutture private rappresentano un calo del 30% rispetto all’anno 2000. L’unica regione in linea con la media Ocse è il Friuli Venezia Giulia, che conta 5 posti ogni mille abitanti, quasi il doppio della media nazionale).
Secondo l’Organizzazione Mondiale della Sanità, l’Italia ha a disposizione 164mila posti letto per pazienti acuti (272 ogni centomila abitanti), dato calato di un terzo dal 1980 a oggi. I posti in terapia intensiva sono invece poco più di 3.700, che diventano 5.300 (8,4 ogni 100mila abitanti) se consideriamo anche le strutture private. Attualmente, sul territorio nazionale, i pazienti ricoverati in terapia intensiva a causa del Covid-19 sono 1.028, di cui 560 nella sola Lombardia.

Minimum history of 40 years of cuts to Italian healthcare
The emergency of these hours is not just the result of contingency, but a structural problem at least four decades old. All the numbers of the Italian healthcare system, still considered among the best in the world
In recent days, the Italian public debate has learned to deal with the term disaster medicine, the scientific field that deals with developing an adequate health response in the face of emergency situations and the consequent scarcity of medical resources. Various journalistic accounts tell of a healthcare system heavily under stress, with intensive care departments on the verge of collapse and painful choices about which patients to intubate.
Added to the alarm raised by the media in recent hours is the apprehension of the mayor of Bergamo, Giorgio Gori, who on Twitter spoke of “patients left to die” because they cannot be treated. At the moment, there is no certain data on the saturation of the Lombardy facilities and numerous medical voices openly exclude similar reconstructions, but in no case would the patients be “left to die”, since even in the absence of ventilators all the necessary care is provided.
What we know with certainty, however, is that the state of Italian healthcare today is not solely the result of contingency. Rather, it is a structural problem, the result of precise public finance choices, which over the course of 40 years have contributed to weakening a health service considered, despite everything, still among the best in the world.
The numbers of Italian healthcare
In 2018, Italy spent 8.8% of its GDP on the national healthcare system, a percentage that drops to 6.5% considering only public investments. We perform worse than the United States (14.3%), Germany (9.5%), France (9.3%) and the United Kingdom (7.5%), but substantially in line with the OECD average, which stands at 6. 6%. Below us only the countries of Eastern Europe, Spain, Portugal and Greece.
In absolute numbers this translates into an outlay for the state of 2,326 euros per person (2,000 less than Germany), a total of 8.8 billion more than in 2010. A growth rate of 0.90%, therefore, which with the average annual inflation at 1.07% translates into a definancing of 37 billion. The Gimbe Foundation calculates that the bulk of the cuts occurred between 2010 and 2015 (Berlusconi and Monti governments), with around 25 billion euros withheld from the finance companies of the period, while the remaining 12 billion served for the implementation of the objectives of public finance between 2015 and 2019 (Letta, Renzi, Gentiloni, Conte governments).
As stated in the annual report of the Court of Auditors, the most important slowdown came from the investments of local authorities (-48% between 2009 and 2017) and spending on human resources (-5.3%), a a combination which in practical terms has repercussions on the quantity and modernization of equipment, as well as on the availability of employed personnel, which fell by 46 thousand units in the period taken into consideration (including 8 thousand doctors and 13 thousand nurses).Failed investments they are felt especially in southern Italy, where all regions (with the exception of Molise) spend less than the national average.
The numbers in hospitals
The most reliable data to help us understand what is really happening in Italian hospitals comes from the statistical yearbook of the National Health Service and is updated to the year 2017. The total beds available in public facilities are 151,646 (2.5 per thousand inhabitants ), which added to the over 40 thousand units included in private structures represent a 30% decrease compared to the year 2000. The only region in line with the OECD average is Friuli Venezia Giulia, which has 5 places per thousand inhabitants, almost double the national average).
According to the World Health Organization, Italy has 164 thousand beds available for acute patients (272 per one hundred thousand inhabitants), a figure that has fallen by a third from 1980 to today. The places in intensive care are just over 3,700, which becomes 5,300 (8.4 per 100 thousand inhabitants) if we also consider private facilities. Currently, on the national territory, there are 1,028 patients hospitalized in intensive care due to Covid-19, of which 560 in Lombardy alone.

Nell’ultima campagna elettorale nell’autunno 2022 Silvio Berlusconi, ancora leader di Forza Italia, aveva promesso che il diritto alla salute sarebbe stato garantito in tutto il Paese e che medici ed infermieri sarebbero stati valorizzati per la loro professionalità, anche con adeguate remunerazioni. Accessibilità ai servizi sanitari e riduzione del fenomeno delle lunghe liste d’attesa per visite ed interventi sono state le sue ultime promesse elettorali. In realtà i governi Berlusconi hanno tagliato la spesa sanitaria già nel suo secondo governo (2001-2005) con il decreto legge n.347 del 18 Settembre 2001 “Interventi urgenti in materia di spese per l’assistenza sanitaria”.
Anche la Sanità, per Berlusconi, era un affare
Silvio Berlusconi, quattro volte Presidente del Consiglio, è morto oggi all’ospedale San Raffaele di Milano. Ne danno notizia le maggiori testate giornalistiche del mondo, dalla BBC alla CNN ad Al Jazeera.
Ne parlano in ogni angolo del globo. Anche il pontefice twitta il suo cordoglio dall’ospedale Gemelli di Roma. Mosca lo definisce un grande statista europeo e un visionario, sono ampiamente note le sue relazioni amichevoli con Vladimir Putin in contrasto con le posizioni atlantiste assunte dai governi che ha guidato.
È considerato un rivoluzionario che ha cambiato profondamente la vita, il costume, la comunicazione, la politica italiana. Ha creato la televisione commerciale, togliendo il monopolio alla Rai, senza canone ma con tanta pubblicità. Ha ideato il centro destra.
Malato di una grave forma di leucemia cronica, si è spento dopo l’ultimo ricovero per un improvviso peggioramento delle stato di salute. Anche se ufficialmente era entrato in ospedale per essere sottoposto ad alcuni accertamenti clinici anticipatamente rispetto al percorso di cura cui era sottoposto, si intuiva che, in ragione dell’età e della chemioterapia, la fine fosse ormai vicina. I funerali di Stato si svolgeranno mercoledì nel Duomo a Milano. La camera ardente sarà allestita a Mediaset.
Il Quirinale lo definisce un protagonista di lunghe stagioni della politica italiana delle istituzioni repubblicane, un leader politico che ha segnato la storia della nostra Repubblica, incidendo su paradigmi, usi e linguaggi. Ottenne consensi così larghi da poter comporre una maggioranza e un governo. La sua leadership ha plasmato una nuova geografia della politica italiana affrontando eventi di portata globale, dalle Torri Gemelle alla lotta al terrorismo internazionale sino agli sconvolgimenti finanziari alla fine del primo decennio del nuovo secolo

Il Presidente Mattarella lo considera una persona dotata di grande umanità e un innovatore nel suo campo, che ha saputo conquistare posizioni di assoluto rilievo nell’industria televisiva e nel settore dei media, ben prima del suo impegno diretto nelle istituzioni.
Silvio Berlusconi è stato infatti un imprenditore di grande successo, un miliardario diventato un controverso e divisivo leader politico italiano, dopo aver formato un partito nuovo. È stato un uomo incredibilmente popolare con una forte personalità. Dal 1994 ha dominato la politica e la cultura italiana. È stato un uomo di luce ed ombra che ha segnato un’epoca ed ha conosciuto la gloria e il declino.
Come per ogni lutto, questa figura celebre e carismatica, molto complessa, merita cordoglio e rispetto. Era un uomo di potere, non solo economico, amato ed odiato a seconda del giudizio politico ed umano. È stato un uomo dei conflitti di interesse che sono stati accomodati. Come uomo che ha lasciato un segno nella storia del Paese, viene oggi ricordato per il buono e meno buono che ha realizzato.
È passato attraverso grandi opere e grandi inchieste, tra processi giudiziari, sentenze, condanne ed imbarazzi nello scenario mondiale. Un politico showman, scrivono i giornali americani. Un libertario e un libertino, tra grandi eventi della storia e scandalose vicende personali. Ha portato a modo suo l'”american dream” e l'”I can” in Italia.

Ma i cittadini italiani devono avere tutti gli stessi diritti in ogni Regione

Berlusconi dichiarava che ci sono molte cose che non funzionano nel nostro sistema sanitario, innanzitutto le liste di attesa che costringono le persone malate a compiere lunghi viaggi da una Regione all’altra per trovare cure immediate e migliori. Lo riteneva inaccettabile.
In realtà i governi Berlusconi hanno tagliato la spesa sanitaria già nel suo secondo governo (2001-2005) con il decreto legge n.347 del 18 settembre 2001 “Interventi urgenti in materia di spese per l’assistenza sanitaria”. Da un rapporto dettagliato della Fondazione Gimbe sui tagli alla sanità nel decennio 2010-2019 emerge che i fondi sono stati sistematicamente tagliati nonostante un crescente e costante aumento del fabbisogno sanitario nazionale.
Si calcola che il massimo dei tagli sia avvenuto tra il 2010 e il 2015, sotto i governi Berlusconi e Monti, ritenuti i primi responsabili di questo dissesto ed imitati dai governi che li hanno seguiti. Essi hanno tolto con varie manovre finanziarie ben 25 miliardi alla salute. Altri 12 ne sono stati tolti nel periodo 2015-2019 con un definanziamento che ha assegnato meno risorse al SSN rispetto ai livelli programmati.
“In dieci anni sono stati sottratti alla sanità pubblica 37 miliardi. Tutti i governi hanno prelevato i fondi destinati alla spesa sanitaria per esigenze di finanza pubblica, sgretolando progressivamente, secondo Gimbe, la più grande opera pubblica mai costruita in Italia. Il Servizio Sanitario Nazionale.”
Salute: il capitolo di spesa pubblica più facilmente aggredibile
Anche la Sanità, per Berlusconi, era un affare, almeno in Lombardia. Con un capitalismo definito compassionevole, ha creato cliniche private, colossi della sanità con milioni di profitti come San Donato, San Raffaele, Humanitas. Si tratta di strutture eccellenti ma non sono il SSN. Possono essere complementari ma non sostitutive. Il diritto alla salute è un bene pubblico che lo Stato dovrebbe costituzionalmente impegnarsi a garantire e a tutelare.
Anche Berlusconi pertanto ha contribuito a smantellare la sanità pubblica italiana che nel 2023, con oltre 40 miliardi di tagli complessivi, appare devastata. Viene imposta ancora austerità.

. E mi ritornano in mente i famosi versi di Manzoni, come epitaffio per Berlusconi, un’iscrizione sepolcrale con il quale si possono celebrare, anche se non è Napoleone, le sue lodi di defunto illustre. Il lungo addio sarà caratterizzato da omaggi ed onori anche se non è stato un santo, né un eroe.

Even healthcare, for Berlusconi, was a business
Silvio Berlusconi, four-time Prime Minister, died today at the San Raffaele hospital in Milan. The major newspapers in the world report it, from the BBC to CNN to Al Jazeera.
They talk about it in every corner of the globe. The pontiff also tweets his condolences from the Gemelli hospital in Rome. Moscow defines him as a great European statesman and a visionary, his friendly relations with Vladimir Putin are widely known in contrast to the Atlanticist positions taken by the governments he led.
He is considered a revolutionary who profoundly changed Italian life, customs, communication and politics. He created commercial television, taking away the monopoly from Rai, without license fees but with lots of advertising. He created the center right.
Suffering from a serious form of chronic leukemia, he passed away after his last hospitalization due to a sudden worsening of his health. Even though he had officially entered the hospital to undergo some clinical tests ahead of the course of treatment he was undergoing, it was clear that, due to his age and chemotherapy, the end was now near. The state funeral will take place on Wednesday in the Duomo in Milan. The funeral chapel will be set up in Mediaset.
The Quirinale defines him as a protagonist of long seasons of Italian politics of republican institutions, a political leader who has marked the history of our Republic, impacting paradigms, uses and languages. He obtained such broad consensus that he was able to compose a majority and a government. His leadership shaped a new geography of Italian politics by addressing events of global scope, from the Twin Towers to the fight against international terrorism up to the financial upheavals at the end of the first decade of the new century.
President Mattarella considers him a person endowed with great humanity and an innovator in his field, who has been able to conquer positions of absolute importance in the television industry and in the media sector, well before his direct commitment to the institutions.
Silvio Berlusconi was in fact a very successful entrepreneur, a billionaire who became a controversial and divisive Italian political leader, after forming a new party. He was an incredibly popular man with a strong personality. Since 1994 it has dominated Italian politics and culture. He was a man of light and shadow who marked an era and knew glory and decline.
As with any loss, this famous and charismatic, very complex figure deserves condolence and respect. He was a man of power, not only economically, loved and hated depending on political and human judgement. He was a man of conflicts of interest that were accommodated. As a man who left a mark on the country’s history, he is remembered today for the good and not so good he accomplished.
It has gone through great works and great investigations, including judicial trials, sentences, convictions and embarrassments on the world stage. A political showman, the American newspapers write. A libertarian and a libertine, between great events in history and scandalous personal events. He brought the “American dream” and the “I can” to Italy in his own way.

It is not easy for those in government to resolve the problems of the healthcare system, both because there are too many doctors missing and because our Constitution attributes a large part of the responsibilities in healthcare matters not to the national State but to the individual Regions. But Italian citizens must have all the same rights in every Region.
Berlusconi declared that there are many things that don’t work in our healthcare system, first and foremost the waiting lists that force sick people to make long journeys from one region to another to find immediate and better care. He found it unacceptable.
In reality, the Berlusconi governments cut healthcare spending already in his second government (2001-2005) with law decree n.347 of 18 September 2001 “Urgent interventions regarding healthcare expenditure”. A detailed report by the Gimbe Foundation on healthcare cuts in the decade 2010-2019 shows that funds have been systematically cut despite a growing and constant increase in national health needs.
It is estimated that the maximum cuts occurred between 2010 and 2015, under the Berlusconi and Monti governments, considered primarily responsible for this disruption and imitated by the governments that followed them. Through various financial maneuvers they have taken as much as 25 billion from healthcare. Another 12 were removed in the period 2015-2019 with a defunding that allocated fewer resources to the NHS than planned levels.

Health: the most easily attackable chapter of public spending
Even Healthcare, for Berlusconi, was a business, at least in Lombardy. With a capitalism defined as compassionate, it has created private clinics, healthcare giants with millions of profits such as San Donato, San Raffaele, Humanitas. These are excellent facilities but they are not the NHS. They can be complementary but not substitutes. The right to health is a public good that the State should constitutionally undertake to guarantee and protect.
Berlusconi has therefore also contributed to dismantling Italian public healthcare which in 2023, with over 40 billion in total cuts, appears devastated. More austerity is imposed.
